If H&H scores PEEK Forms (no software purchased):

1-499 Quantity = $2.50/each
500+ Quantity = $2.25/each

If institution scores PEEK (A one time $10 software purchase is required. A Scantron 8000 series scanner with two read heads or an NCS scanner in 3000 emulation mode is also required by the institution to score PEEK.):

1-499 Quantity = $1.75/each
500+ Quantity = $1.50/each
